Before you can begin to tutor, you must complete certain documentation required by the State of Pennsylvania for all individuals who will be working with youth.
Required Clearances:
- Act 33 Pennsylvania Child Abuse History Clearance - access form at http://www.dpw.state.pa.us/ Under "DPW Keywords", select Child Abuse Clearance Forms
- Form SP 4-164 Pennsylvania State Police Request for Criminal Record Check - online option at https://epatch.state.pa.us/Home.jsp
- FBI Federal Criminal History Records for Prospective Pennsylvania Department of Education (PDE) Employees - must FIRST pre-register online at https://www.pa.cogentid.com/index_pde.htm or over the phone at 1-888-439-2486, Monday-Friday, 8AM to 6PM EST
- Proof of Liability Insurance - Education majors MUST apply through the Pennsylvania State Education Association (PSEA) online at http://www.psea.org/
- Must be current - within one year
- Must be the Intracutaneous Mantoux Method
- Must provide proof (signed official document) from Primary Care Provider or Campus RN
